LHA is assisting CareerSource Polk with the distribution of the Florida Reemployment Assistance Application.  Copies of the application in English, Spanish and Creole are available for download from our website.  Please click on the link below.  Paper copies of the application are also available at the following LHA properties:  Renaissance at Washington Park, John Wright, Villas of Lake Bonnet, Colton Meadows and the LHA Main Office. 

Completed applications may be mailed directly to the Florida Department of Economic Opportunity (DEO).  The DEO address is on the application.  Also, completed applications may be hand delivered or dropped off at the Lakeland Career Center (LCC) during normal business hours.  The LCC address is 309 N. Ingraham Avenue, Lakeland, FL  33801.
